Help Me Build My Address Book!

In an effort to live a more beautiful life, I will be deleting all of my social media profiles. 
My biggest concern is to lose touch with those whom instagram is our only line of communication, and I realized my website has a wonderful way of collecting information from all of you! 

If you'd all please help me build my address book in case I need to reach you, I would really appreciate it! 
Also, please give me your physical address so I can send my first annual Christmas card to you this year! 

Also, I will send you an actual post card as a thank you immediately  :-)
